BE IT RESOLVED, that the Board of Hennepin County Commissioners-HHS Governing Body approves the initial appointment applications for medical staff clinical privileges contained in the December 2025 Credentialing report; and
BE IT FURTHER RESOLVED, that the Board of Hennepin County Commissioners-HHS Governing Body receives, as information, completed Focused Professional Practice Evaluation plans (FPPE) and the medical staff resignations contained in the December 2025 Credentialing report

Background:
The December 2025 Credentials Committee report reflects that the standard medical staff credentialing process at Hennepin Healthcare System (HHS) has been appropriately followed. The process begins when a current or prospective medical staff member submits an application. A credentialing specialist reviews and verifies the information, flagging any notable items for further evaluation. The application is then reviewed and recommended for appointment by the Department Chair and the Medical Staff Credentials Committee.
At the Credentials Committee, HHS medical staff physicians and providers conduct an in-depth review of each applicant. Requests for modifications to clinical privileges follow a similar process: the credentialing specialist verifies the request, and the Department Chair and Credentials Committee review it. A committee member provides a detailed assessment, and the full committee discusses and votes on recommendations. Following committee approval, the recommendations are reviewed and endorsed by the Medical Executive Committee
