BE IT RESOLVED, that Agreement A2311988 with the U.S. Department of Housing and Urban Development Continuum of Care Program to accept grant funds for the provision of coordinated entry and assessment services for people experiencing homelessness during the period November 1, 2023 through October 31, 2024, in the receivable amount of $165,115, be approved; that the Chair of the Board be authorized to sign the Agreement on behalf of the county; and that the Controller be authorized to disburse the funds as directed; and

BE IT FURTHER RESOLVED, that sponsorship and acceptance of grant funding for this program by the Hennepin County Board of Commissioners does not imply a continuing funding commitment by Hennepin County for this program when grant funds are no longer available.

Background:
Matrix was first awarded this grant for Coordinated Entry System (CES) assessment services in 2017, with subsequent annual renewals by the US Department of Housing and Urban Development (HUD). However, Matrix closed its operation of assessment services for the CES, funded by this grant, effective June 1, 2022, and it was transferred to Hennepin County, in order for these services to continue in the Hennepin County community. This transfer was approved as Amendment 1 to Agreement A2211275 by the board on July 12, 2022 (Board Resolution 22-0261).

In 2022 Hennepin County applied for and was recently awarded these renewal grant funds for the program year of November 1, 2023 through October 31, 2024.
