BE IT RESOLVED, that the County Administrator be authorized to negotiate Amendment 3 to Lease Agreement A142072 with 800 West Broadway LLC for 2,748 square feet of rental space located at 800 West Broadway, Minneapolis, extending the contract period through February 28, 2032, in the estimated amount of $61,762 first year rent and operating costs; that following review and approval by the County Attorney's Office, the Chair of the Board be authorized to sign the amendment on behalf of the County; and that the Controller be authorized to disburse funds as directed.

Background:
NorthPoint Health & Wellness Center, located at 2220 Plymouth Ave. N. in North Minneapolis, is a federally qualified health center. The health center operates as a partnership between NorthPoint's Community Board and Hennepin County Board of Commissioners. NorthPoint, as a public entity community health center, is recognized as an innovative model of shared ownership operating within an integrated health and human services system of care.
NorthPoint's neighborhood sites are strategically located in areas to serve populations with multiple barriers and health disparities that prevent or limit their ability to obtain services at the main campus.
NorthPoint has leased approximately 2,748 square feet of clinic space at 800 West Broadway, Minneapolis since December 2016. This NorthPoint clinic serves the medical and dental care needs of area residents.
