BE IT RESOLVED, that Amendment 1 to Agreement A2111075 with Minnesota Department of Human Services to continue the provision of Transition to Community Services, extending the end date from December 31, 2024 to June 30, 2025 and increasing the receivable amount by $500,000 for a new total receivable amount of $1,802,000, be approved; and that the Chair of the Board be authorized to sign the amended agreement on behalf of the county; and that the Controller be authorized to disburse the funds as directed; and

BE IT FURTHER RESOLVED, that sponsorship and acceptance of grant funds for this program by the Hennepin County Board of Commissioners does not imply a continued funding commitment by Hennepin County for this program when grant funds are no longer available.

Background:
The original agreement was a two-year agreement with the Minnesota Department of Human Services Mental Health Division to improve the transition from Anoka Regional Treatment Center (AMRTC) and Minnesota Security Hospital - St. Peter (MSH) to a community placement for patients who have complex mental health treatment and other service needs.

The grant enables customized discharge planning and allows for more timely discharges. Funds address system gaps that pose barriers to discharge. The majority of the grant allocation has been used to purchase community-based services for between 8 to 10 individuals who have a court order for payment.
