BE IT RESOLVED, that Amendment 3 to Agreement A2111075 with Minnesota Dept of Human Services to continue the provision of Transition to Community Services, extending the end date to December 31, 2026, and increasing the receivable amount by $275,000 for a new total receivable amount of $2,902,500 be approved; and that the Chair of the Board be authorized to sign the amended agreement on behalf of the county; and that the Controller be authorized to disburse the funds as directed; and

BE IT FURTHER RESOLVED, that sponsorship and acceptance of grant funds for this program by the Hennepin County Board of Commissioners does not imply a continued funding commitment by Hennepin County for this program when grant funds are no longer available.

Background:

The original agreement began in January of 2022 and was a two-year agreement with the Minnesota Department of Human Services Mental Health Division to improve the transition from Anoka Regional Treatment Center (AMRTC) and Minnesota Security Hospital - St. Peter (MSH) to a community placement for patients who have complex mental health treatment and other service needs. Amendment 1 was approved by the board on June 25, 2024, increasing the agreement by $500,000 and extending the term by six months. Amendment 2 was approved by the board on November 19, 2024, increasing the agreement by $825,500 and extending the terms of the agreement to 06/30/2026.

The grant enables customized discharge planning and allows for more timely discharges. Funds address system gaps that pose barriers to discharge. The majority of the grant allocation has been used to purchase community-based services for individuals who have a court order for payment. Some funding is being used for the Bed Hold program which has seen significant success in supporting transitions from the Forensic Mental Health Program (FMHP). As of 2025, 11 residents were able to discharge into the community with the support of Bed Hold funding, more than double the number from the previous year. This increase reflects strong collaboration between case managers, supervisors, and residential providers, as well as the effectiveness of the Transition to Community grant in removing barriers to timely discharge.

 

This request supports the county’s identified priorities and goals for the disparity elimination Health domain by focusing on achieving health equity where every person feels they have the opportunity to attain their highest health potential, and no one is disadvantaged from achieving this potential because of social position or other socially determined circumstances.
