BE IT RESOLVED, that Amendment 4 to Agreement PR00000886 with Delta Dental of Minnesota, Hennepin Health’s dental network and a provider of administrative services, extending the end date to April 30, 2028; increasing the not to exceed amount by $2,000,000 for a new total not to exceed amount of $34,250,000, amending the TERM OF THE AGREEMENT section and the RIGHT TO AUDIT section; Exhibit II, PRODUCTS AND ADMINISTRATIVE SERVICES; Exhibit IV, PAYMENT FOR SERVICES; EXHIBIT V, SERVICE LEVEL AGREEMENTS; EXHIBIT VI, CHANGE MANAGEMENT be approved; the Chair of the Board be authorized to sign the Amendment on behalf of the County; and that the Controller be authorized to dispense funds as directed.

Agreement PR00000886 with Delta Dental of Minnesota, Hennepin Health’s dental network and a provider of administrative services expires April 30, 2025.
Current Request: Approval of Amendment 4 to Agreement PR00000886 will:
• Extend end date through April 30, 2028
• Update the Corrective Action Plan language
• Add accuracy of data certification language
• Add contracts with DHS language
• Update the dental provider network expansion efforts language
• Update the Administrative Fee language
• Add the Regulatory Changes language
• Update the Performance Guarantee language
• Update the language of the Agreement to comply with DHS contract changes
The agreement with Delta Dental of Minnesota includes the pass through of medical expenses. This request increases the NTE by $2,000,000. The new NTE is $34,250,000.
This board action request aligns with Hennepin County disparity reduction efforts to improve access to culturally specific/sensitive health services and increase community well-being.
