BE IT RESOLVED, that Agreement A2311983 with the Centers for Disease Control and Prevention (CDC) funding for Closing the Gap with Social Determinants for Black and African American Communities within Hennepin County, part of the Health Promotion Program from September 30, 2023 through September 29, 2024, in a receivable amount of $125,000 be approved; that the Chair of the Board be authorized to sign the agreement on behalf of the county; and that the Controller be authorized to accept and disburse funds as directed; and

BE IT FURTHER RESOLVED, that a supplemental appropriation be made to increase the Public Health’s 2023 adopted budget by $31,250 and

BE IT FURTHER RESOLVED, that sponsorship and acceptance of grant funding for this program by the Hennepin County Board of Commissioners does not imply a continued funding commitment by Hennepin County for this program when grant funds are no longer available.

Background:

The Hennepin County Social Determinants of Health’s (HCSDOH) planning initiative will develop a comprehensive action plan to address the social determinants of health within the Black/African American communities in Hennepin County. This planning grant will build upon community engagement conducted in 2022 to identify community-wide approaches that will reduce chronic disease disparities and improve population health by focusing on community-clinical linkages and social connectedness. HCSDOH will center its efforts within North Minneapolis, Brooklyn Park, and Brooklyn Center. The Leadership Team will be comprised of multisectoral partners including healthcare systems, Federally Qualified Health Centers, community-based organizations, and community members. 

This action supports the county’s disparity reduction efforts in the health domain by addressing the social determinants of health within the Black/African American communities in Hennepin County.
