BE IT RESOLVED, that Amendment 3 to Agreement A2311962 with the Substance and Mental Health Services Administration Early Diversion; extending the end date to September 29, 2027, and increasing the receivable amount by $330,000 for a new total receivable amount of $1,320,000 be approved; that the Chair of the Board be authorized to sign the agreement on behalf of the county; and that the Controller be authorized to accept and disburse funds as directed; and
BE IT FURTHER RESOLVED, that sponsorship and acceptance of grant funding for this program by the Hennepin County Board of Commissioners does not imply a continued funding commitment by Hennepin County for this program when grant funds are no longer available; and
BE IT FURTHER RESOLVED, that acceptance of this award in no way reflects acceptance of terms or conditions that are legally unenforceable against Hennepin County.

Background:
This Substance Abuse and Mental Health Services Administration grant is a five-year funding opportunity, with each year of funding made available in the amount of $330,000. Grant allocations for subsequent years are awarded on an annual basis.
The Hennepin County Behavioral Health Center (BHC) at 1800 Chicago Avenue, Minneapolis, has expanded its services and hours with support from the SAMHSA Early Diversion grant, as part of a broader initiative to divert individuals with mental illness or co-occurring disorders away from the criminal justice system and into community-based care.
By expanding access to low-barrier, comprehensive supports available at the BHC, Hennepin County aims to improve outcomes and increase access to services for residents who are disproportionately impacted by mental health and substance use challenges - particularly those who frequently interact with emergency departments, hospitals, and the criminal justice system.
Through this funding, the BHC broadened its reach, strengthened program offerings, and expanded service availability for Hennepin County residents. With a September 30th goal of serving 1,000 residents, the BHC exceeded this benchmark with service to more than 1,100 residents. An analysis of residents served at BHC shows more than a 50% decrease in inpatient hospitalizations in the six months following. As of June 30th, the program achieved 200% of its annual screening target within the first three quarters, and the Downtown Improvement District (DID) Social Worker program reached 150% of its target during the same period by supporting more than 1,500 residents. In addition, the program expanded timely access to behavioral health and opioid treatment by offering walk in crisis assessments, brief diagnostics, direct linkages to outpatient opioid treatment providers, and coordinated warm handoffs that support continuity of care. The BHC also continues to leverage resources within partner healthcare settings to divert individuals from unnecessary emergency department utilization by providing care coordination, addressing non-medical needs, and connecting individuals to appropriate community supports. Collectively, these efforts enhanced access, stabilization, and sustained engagement across the behavioral health continuum.
